Another new addition to the online lean world — a blog at Productivity Press. It's written by Ralph Bernstein, the former editor of the Lean Manufacturing Advisor newsletter.
Here is a story they posted about an $11 million dollar mistake that should have been errorproofed. This chemical plant had a two week shutdown due to someone hitting a line stop button. A bit different than pulling the andon at Toyota, eh? It's more likely that the company is looking for “who” to blame (or is it “whom?”) rather than “why” that happened.
